Application of Vathin single-use flexible video bronchoscope in ICU

In 1879, German scientist Gustav Killian removed a foreign body from the trachea using an esophagoscope, marking the first time in history that a rigid endoscope was used to enter the trachea for rigid bronchoscopy. Since then, the bronchoscopy has evolved from rigid bronchoscopy and fiberobronchoscopy to electronic bronchoscopy, with a history spanning over 120 years.

With the development of science and technology, the use of bronchoscopes has not only been limited to the diagnosis and treatment of respiratory diseases, but also be a "golden eye" deep into the lungs, which has played a superior role in the application of ICU. 

 It can directly observe the lesions of the trachea and bronchus, eliminate airway obstruction caused by respiratory secretions, bronchial foreign bodies, and severe hemoptysis, and play a unique role in emergency treatment for critical patients. It is an essential "good helper" for ICU doctors.


Clinical application requirements of bronchoscopy


I. Application of bronchoscopy in difficult intubation and adjustment of endotracheal intubation

Patients with severe head trauma, severe maxillofacial injury, difficulty in opening the mouth, and cervical spine injury who develop respiratory failure and cannot tolerate endotracheal intubation under the guidance of a transoral laryngoscope should be preferentially selected for early airway opening under the guidance of a bronchoscope via a transnasal endotracheal tube, in order to win the chance of successful rescue.
In addition, some patients have obesity, short and thick neck, micrognathia, abnormalities in the oropharynx and larynx, especially those with neck masses compressing the trachea, or with a large amount of secretions in the oropharynx resulting in unclear glottis exposure and difficulty intubating, can undergo tracheal intubation under the guidance of bronchoscopy, which can clear local secretions, unclog the airway, improve oxygenation, and quickly restore oxygen supply to the patient.
Specific operation:After lubricating the endotracheal tube sufficiently, it is placed on the bronchoscope. The bronchoscope is first inserted into the patient's trachea through the nose, and the endotracheal tube is inserted using this as a guide. If there is resistance, the tube can be rotated appropriately, and the depth of insertion can be adjusted based on the microscopic observation.

II. The role of bronchoscopy in the etiological diagnosis and treatment of severe pulmonary infections

Lung infection is a major health problem, and pneumonia and related lung infections are one of the major causes of death. The application of bronchoscopy technology has made great progress in the diagnosis and treatment of lung diseases, and has become an indispensable and important means for clinical diagnosis and treatment of lung diseases.

For some patients with severe pulmonary infections and ventilator-associated pneumonia, including those with immune deficiency and pulmonary infections, empirical anti-infective treatment is not effective, with a low sputum culture positive rate and poor specificity, which make it difficult to achieve "precise" and "timely" anti-infective treatment.

In contrast, the bacterial culture of the secretion, protective brush examination and alveolar lavage fluid collected by bronchoscopy under sterile operation has high sensitivity and specificity, and has a strong guiding role for the application of clinical anti-infective drugs.

At the same time, the operation of bronchoscopy under direct vision can reach the bronchial segments of the diseased lobe, and can aspirate a large amount of purulent or viscous sputum thoroughly; meanwhile, it can play a local cleaning role through pulmonary lavage; combined with systemic medication, it can achieve more satisfactory therapeutic effects.

Targeted drugs based on clinical etiological examination can significantly improve the efficacy, avoid antibiotic abuse, reduce the emergence of drug-resistant bacteria, and save medical resources.


Ⅲ. The Role of Bronchoscopy in Airway Management and Treatment of Atelectasis Patients
with advanced age, frailty, multiple comorbidities (especially stroke), and chronic obstructive pulmonary disease have more airway secretions after lung infection; at the same time, their ability to spontaneously expectorate sputum is weak or even lost, resulting in poor drainage of sputum, which can lead to airway obstruction, atelectasis, and even respiratory failure in severe cases. However, the effects of back-patting and airway humidification are usually not obvious.

Bronchoscopy can enter the lower respiratory tract of patients, and can generally reach the subsegmental bronchial opening to suction sputum, remove sputum plugs and sputum crusts. It can even provide local saline or drug lavage therapy, combined with back-patting to dislodge sputum to achieve lung recruitment.

Moreover, there is also the rescue of drowning patients, which can be achieved by fully suctioning out the water and secretions in the bronchial tube through bronchoscopy while completing endotracheal intubation, quickly alleviating the patient's symptoms.


IV. The role of bronchoscopy
in the diagnosis and treatment of hemoptysis  Hemoptysis is a common clinical emergency in respiratory system that may result in asphyxia, shock, or even death if not treated in time. Due to its bendability and wide field of view, bronchoscopy can access the third-grade bronchus and observe all four-grade bronchus. Direct visualization under bronchoscopy can identify the bleeding site and condition, to take targeted treatment measures. Therefore, it is an extremely effective examination method for determining the bleeding site and identifying the cause of bleeding in patients with hemoptysis, especially in patients with negative chest imaging.

V. Application of bronchoscopy
in the diagnosis of tracheoesophageal fistula Some elderly patients with severe illnesses, poor nutritional status, and long-term tracheal intubation, occasionally have coughing during nasal feeding or the inhalation of gastric contents such as nasal feeding liquid from the airway, which requires exclusion of tracheoesophageal fistula. Suspected that the pressure of the tracheal intubation balloon is too high or the airway wall is compressed for a long time, which may lead to tracheoesophageal fistula, the general imaging examination often cannot diagnose it. It can be observed under bronchoscopy. Smaller tracheoesophageal fistula can be confirmed by methylene blue test. After taking 1:50-1:100 methylene blue solution orally, observe the posterior wall of the trachea under bronchoscopy. If there is blue pigment, it is positive, which provides diagnostic basis for further treatment.

VI. The application of bronchoscopy in the diagnosis of airway stenosis and new organisms in trachea

Among severe patients, long-term intubation patients may experience granulation tissue hyperplasia due to repeated friction of the intubation distal end on the airway inner membrane, partially blocking the airway and causing dyspnea; or patients with previous tracheal intubation and scar-prone skin may experience balloon compression of part of the mucosa, resulting in airway obstruction and severe cases may even lead to asphyxia; the more common cause of benign airway stenosis in China is tuberculosis of the bronchial inner membrane. Bronchoscopy can confirm or rule out this diagnosis.
